1. Assumption of Risk

I understand that participation in museum activities, exhibits, and programs may involve physical activity and interaction with materials, equipment, and other participants. I acknowledge that these activities involve risks, including but not limited to, minor injuries (such as scrapes, bumps, or falls) or, in rare cases, more serious injury.
I voluntarily assume all risks associated with my child’s participation in activities at Kids Discovery Museum.

2. Release of Liability

In consideration of my child being allowed to participate in museum activities, I, on behalf of myself, my child, and our heirs, executors, and assigns, hereby release, discharge, and hold harmless Kids Discovery Museum, its employees, volunteers, agents, officers, and directors from any and all claims, liabilities, demands, or causes of action arising out of or related to my child’s participation in museum activities or use of museum facilities.

3. Medical Treatment Authorization

In the event of an emergency, I authorize museum staff to obtain necessary medical treatment for my child. I agree to assume responsibility for any medical costs incurred.

4. Acknowledgment of Understanding

I have read this waiver and release, fully understand its terms, and sign it voluntarily. I understand that by signing this form, I am giving up certain legal rights, including the right to sue the museum for any injuries or damages my child may sustain while participating in museum activities.
